Announcement, New Procedure of “Receiving and Settling Insurance Claim”

In accordance with Guideline on Receiving and Settling Insurance Claim issued by Insurance Regulator of Cambodia on 16th January 2023, Fortune Life Insurance Plc. would like to confirm new procedures of receiving and settling insurance claim following the above-mentioned guidance as per stated in the Endorsement letter to insurance policies below.

Endorsement Letter to Insurance Policy of Basic Plan
and/or Insurance Policy of Rider(s) attached to the Basic Plan

According to Guideline on “Receiving and Settling Insurance Claims” above and for the purpose of promote customers’ trust, Fortune Life Insurance Plc. would like to amend “Procedure of Receiving and Settling Insurance Claims” for all customers who have bought Insurance Policies from our company. This new procedure is effective date from January 16, 2023 onwards and will replace the existing procedure described in Insurance Policy and/or Insurance Policy of the Rider(s) attached to Basic Plan. Other Terms and Conditions not amended by this new procedure will remain in force. New procedure of Receiving and Settling Insurance Claims is described as below:

1. Notification on Death, Total and Permanent Disability, or Diagnosis of Critical Illnesses under the coverage of Insurance Contract
Life Assured or Beneficiary shall notify immediately to Fortune Life in case of above incidents via methods as follows:
      a. Notify directly,
      b. Telephone,
      c. Electronic mail or message,
      d. Written letter.
After receiving notification of any incident above, Fortune Life will respond to Life Assured or Beneficiary to confirm the receipt of the notification of such incident above within 24 (twenty-four) hours upon the receipt of the notification.

2. Insurance Claim Request
Fortune Life will provide the insurance claim request form and a list of required documents as per the company’s format to Life Assured or Beneficiary (Insurance Claimant) who suffers loss or damage in accordance with the insurance contract within 24 (twenty-four) hours upon the receipt of the notification above.
In this regard, Fortune Life will explain the insurance claimant about how to complete the insurance claim request form and preparation of related documents.
In case the insurance claim request consists of insufficient documents or the completion of insurance claim request form is incorrect, Fortune Life will notify the insurance claimant in writing or via electronic message by specifying the insufficiencies within 3 (three) working days after the company receives the insurance claim request form.

3. Duration of Insurance Claim Settlement
When there is an insurance claim request, Fortune Life will respond to the Insurance Claimant about rejection or approval on the insurance claim in writing within 15 (fifteen) working days after the company receives the complete insurance claim request form and sufficient related documents, or upon the receipt of sufficiently trusted evidences to prove that the insurance claim request is truly correct. The sufficient evidence for insurance claim decision refers to the completion of insurance claim assessment process.

4. Insurance Claim Payment
Insurance claim payment will be made to the insurance claimant via cheque, bank transfer, or other suitable methods within 3 (three) working days after insurance claimant agrees to accept the claim payment.
